Is this form actually anonymous?

Yes. Toggle "anonymous mode" and Foldr strips IP, user agent, and submitter identity before the response hits your dashboard.

Can I filter by department without re-identifying?

Yes. Cohort filters work on aggregated groups only — results are hidden when a cohort is smaller than 5 respondents to protect anonymity.

How often should we run this?

Quarterly is standard. Monthly pulses can work but require shorter surveys (3-5 questions) to avoid fatigue.
